Silent Contamination: The Rising Threat of Antibiotic-Resistant Escherichia coli in Philippine Groundwater

Access to safe drinking water remains a critical public health concern in the Philippines, where many communities rely on untreated groundwater sources such as artesian wells. This case study examines the emergence of multidrug-resistant (MDR) Escherichia coli in groundwater systems and its implications for environmental and human health. Using a literature review approach, the study synthesizes findings from Philippine-based and international research, with primary data drawn from artesian wells in Los Baños, Laguna. Results indicate the presence of E. coli in contaminated water sources, with isolates exhibiting resistance to multiple antibiotics, including cefoperazone, meropenem, ofloxacin, and trimethoprim-sulfamethoxazole. These findings highlight groundwater as both a pathway for microbial contamination and a reservoir for antimicrobial resistance genes. Key contributing factors include fecal contamination, antibiotic misuse, environmental reservoirs of resistance, and inadequate monitoring systems. Anchored in the One Health framework, the study emphasizes the interconnectedness of environmental, animal, and human health in addressing antimicrobial resistance. The findings underscore the urgent need for strengthened water quality monitoring, improved sanitation infrastructure, and the implementation of integrated, cross-sectoral strategies to mitigate the spread of antibiotic-resistant bacteria in water systems.
