SRS205 - Review of trans meatal myringoplasty cases

Abstract Objective Myringoplasty is performed for chronic otitis media. The postaural approach is favoured. However, the transmeatal approach offers reduced operative time with fewer complications and enhanced healing rate. Aim of the study is to review effectiveness of graft take up, complications and operative time for transmeatal approach of myringoplasty. Methods A retrospective analysis. A group of 24 myringoplasty procedures was performed in 23 patients over 18 years of age (mean age 53) with a dry central perforation including subtotal perforations. The study was done between November 2021 and May 2022. All patients underwent a transmeatal myringoplasty approach using tragal perichondrial graft. None of the operated ears were packed with dressing postoperatively. The size of perforation, operating time, healing rate, hearing improvement and complications were analysed. One patient had a bilateral myringoplasty procedure performed simultaneously. All patients were followed up at 1 week and 6 weeks postoperatively with an audiogram. Results Good healing rate was observed in 91% of patients and postoperatively, significant air bone (AB) gap closure in audiogram was seen in 83%. The results were comparable with the results available in literature. Conclusions Transmeatal myringoplasty can be performed in all cases irrespective of size of perforation. Operative time is significantly reduced, so it is cost effective in the era of theatre utilisation. Healing rate is good, and complication rates are lower. There is no pack, so it reduces the pain and anxiety of pack removal. Overall, a transmeatal approach confers favourable outcomes comparable to the conventional approach for myringoplasty.
