A clinical case of surgical treatment of serous ovarian cystadenoma in a woman with Prader—Willi syndrome

Background. Prader—Willi syndrome (PVS) is a rare hereditary disease with multiple clinical manifestations, including obesity, endocrine disorders, reproductive system disorders, and cognitive disorders. Gynecological pathology in patients with PVS has not been studied sufficiently, and the available scientific data on the development of ovarian tumors are extremely limited. The main method of treating benign ovarian growths is surgical, while the amount of surgery depends on the individual characteristics of the woman, the data of the somatic and gynecological anamnesis, and the size of the formation. Objective. In this clinical case, we want to demonstrate a case of successful surgical removal of ovarian formation in a patient with PVS and a burdened somatic status. Material and methods. In the above clinical case, in a 30-year-old patient with PVS and a burdened somatic anamnesis, ultrasound diagnostics revealed the formation of the right ovary with a total volume of 240 mm3, ORADS-3. Previously, the patient had repeatedly contacted third-party organizations, where surgical intervention was refused. Taking into account the initial data of the patient, it was decided to perform surgical treatment in two stages: hysteroscopy with diagnostic curettage of the endometrium and endocervix; laparotomy, adnexectomy on the right. Results. As a result of an intraoperative assessment of the formation of the right ovary, it was decided to expand the scope of surgical intervention: perform a biopsy of the left ovary and large omentum. Histological examination revealed a serous cystadenoma of the right ovary with focal epithelial proliferation. Conclusion. The present observation highlights the importance of a personalized, multidisciplinary approach to the management of patients with PVS, the need for oncological alertness and timely diagnosis to identify gynecological pathology in this category of patients.
