Patient satisfaction after penile prosthesis implantation in men aged ≥80 years: a single-center retrospective descriptive study

Penile prosthesis implantation is an established treatment for medically refractory erectile dysfunction, but patient-reported outcomes in men aged ≥80 years remain underexplored. This retrospective single-center study descriptively evaluated satisfaction, functional device use, and postoperative complications among men aged ≥80 years who underwent penile prosthesis implantation between January 2015 and September 2025. Patients aged 75-79 years were included only to provide clinical context rather than to establish comparative equivalence. Satisfaction was assessed during in-person follow-up using a single non-validated 5-point ordinal item. Among 36 included patients, 13 were aged ≥80 years and 23 were aged 75-79 years. In the ≥80-year group, mean satisfaction was 4.54 ± 1.13, median satisfaction was 5.0 (interquartile range 5.0-5.0), 10 of 13 patients (76.9%) selected the maximum score, 11 of 13 (84.6%) reported full functional use, and 3 of 13 (23.1%) experienced any recorded complication. Contextual between-group estimates were imprecise: the mean satisfaction difference was -0.24 points (95% confidence interval -0.95 to 0.47), and the risk difference for maximum satisfaction was -5.7 percentage points (95% confidence interval -34.8 to 19.0). The clustering of scores at 5 indicates a substantial ceiling effect that limits discrimination by the satisfaction item. Selected men aged ≥80 years reported favorable satisfaction and functional use; however, the small, selected cohort, short and variable follow-up, non-validated single-item measure, and wide confidence intervals preclude conclusions regarding equivalence or broader effectiveness.
