Towards a Sonographer Career Framework: A Narrative Review of Career Frameworks in Health Professions

ABSTRACT Sonographers in Australia and Aotearoa New Zealand (NZ) currently lack a dedicated career framework (CF), despite the existence of such frameworks in other health professions. Distinct from a competency framework, a CF provides a structured model for career progression, capability development and workforce sustainability. This review aimed to identify the core and supporting components of a CF tailored to sonography in Australia and NZ, by analysing frameworks across medical imaging, allied health, nursing and midwifery. A scoping review was conducted between February 2024 and July 2025. White and grey literature searches were performed using academic databases, professional organisation websites and government sources across Australia, NZ, the United Kingdom, Canada and the United States. Frameworks published in English after 2000, aligned with AQF/NZQF level 7 or higher, and endorsed by recognised bodies were included. Data were extracted on profession, publication details, domains of practice, career levels and components relevant to sonography. Nineteen frameworks were identified, with 15 meeting inclusion criteria. Two core components: domains of practice and career levels were consistently present. Supporting components such as capability descriptors, career level descriptors and professional titles were also identified, offering insights for developing a Sonographer Career Framework (SCF). This review provides a foundation for developing a SCF that supports professional growth and workforce sustainability. Future work should focus on co‐designing the framework with stakeholders to ensure relevance and long‐term impact.
