A Case Report of Low-dose Baclofen-induced Severe Encephalopathy in a Chronic Kidney Disease Patient

Case RepoRtfor close monitoring and correction of severe hypokalemia. His electrocardiogram (ECG) and echocardiogram (ECHO) did not show any new changes. He had an unremarkable course in ICU, received intravenous (IV) potassium correction with close monitoring, and was transferred to the ward in the next 24 hours for further care.In the ward, with persistent hiccups, he was treated with oral baclofen 10 mg twice daily along with other supportive measures. His serum electrolytes and serum creatinine were monitored serially. On day 3 of admission, he became drowsy and was unable to recognize family members. He was shifted to the medical intensive care unit (MICU) for further evaluation and management in view of encephalopathy. In the MICU, over the next few hours, his sensorium further dropped, suspecting an ongoing nonconvulsive seizure, IV lorazepam 2 mg was given.ABG (arterial blood gas) analysis showed mild metabolic acidosis (pH: 7.40, pO2: 89 mm Hg, pCO 2
