Dear Friends,We are nearing our annual congress which will be held in the end of February in Chennai.During this occasion, the author of the best case report from the year 2025 will be felicitated.I was given the onerous task of choosing the best case report.Among the various excellent case reports the one which was appealing was the use of next-generation sequencing to identify pathogenic microbes in a pneumonia patient. This case highlights the use of rapid diagnostics in streamlining antibiotic and, if used properly with antibiotic stewardship, will decrease antibiotic load in ICU which may lead to a decrease in antibiotic resistance, the biggest threat facing all of us.I was also asked to prepare a yearly case report on our journal for the congress.Report card of IJCCR was presented in the recently concluded executive committee meeting of ISCCM.In this report the indexing of the journal in the directory of open access journal was highlighted as a step towards Medline indexing.Other suggestions for improvement of the journal were made like auto-appreciation certificate for reviewers, mandatory case report submission from students of various courses run by ISCCM.Case reports can be used as laptop-based long or short cases during ISCCM examination, collection of all cases on a particular theme like poisoning and publishing as a handbook of case reports, periodic feedback and increasing the number of issues and pages of the journal. In this issue of the journal a case of snake envenomation presenting as acute stroke has been described.Sickle cell crisis is occasionally encountered in our population and a case of vaso-occlusive crisis due to this pathology has been described.Shigella dysentery, a common cause of community-acquired diarrhea, is usually mild, but may become an emergency with multiorgan dysfunction even requiring extracorporeal support, is described.Allergic reactions to drugs mainly analgesics are frequently encountered.Such a reaction in pregnancy with propyphenazone has been described.Baclofen is used for various ailments like hiccough, muscle spasticity, etc.Its use in chronic kidney disease with side effects is described in this issue.Various irritative medications like vasopressors, cordarone, etc. are used peripherally and extravasation can cause severe tissue necrosis.Early detection of extravasation by ultrasound is described in a case report.Melioidosis is being increasingly recognized as a cause of community-acquired pneumonia, and it can present with protean manifestation. Its presentation with orbital cellulitis and brainstem infarction is described in a case report.Various case reports on hemophagocytic syndrome have been described earlier in this journal, an atypical case with neuro symptoms is described in this issue of the journal.I hope this plethora of case reports will be a productive reading experience for you.
