Regenerative Endodontic Treatment of An Immature Permanent Tooth with Apical Periodontitis

Objective: Regenerative endodontic treatments are applied to permanent teeth with incompleteroot development, typically when the root formation has completed approximately two-thirds of itsdevelopment. This case report aims to present the regenerative endodontic treatment of animmature permanent tooth with apical periodontitis. Case Report: A 12-year-old patient presented to our clinic with complaints of spontaneousnocturnal pain in tooth #35. Clinical examination revealed percussion sensitivity, and periapicalradiographic evaluation confirmed apical periodontitis. Regenerative endodontic treatment wasconsidered for the patient. In the first session, the infected pulp tissue was extirpated under asepticconditions, and the root canal was irrigated with 20 mL of 2% sodium hypochlorite and 0.9% salinesolution, following the first protocol of regenerative endodontic treatment. In the second session, two weeks later, the root canal was irrigated with 20 mL of 17% EDTA solution and 0.9% salinesolution. Apical bleeding was then induced, and coronal sealing was achieved with a 3-mm layer ofMTA. Permanent restoration was completed using composite material. However, during a follow-upsession, a fracture was observed in the initial restoration, and the restoration was replaced with astainless steel crown. At the 6-month and 1-year follow-ups, continued root development wasobserved, and the symptoms showed both clinical and radiographic resolution. Conclusion: The reported case demonstrates that regenerative endodontic treatment procedurescan be successfully applied to immature necrotic permanent teeth. Further studies on this treatmentapproach are recommended in the literature.
