Effective Sialendoscopy and Duct Transposition of Chronic Parotid Sialadenitis: A Case Report

Introduction Chronic sialadenitis is a recurrent inflammatory condition of the salivary glands. It is often associated with salivary duct obstruction caused by stones, strictures, or other anatomical abnormalities. In severe cases, surgical intervention may be required to alleviate symptoms. Case Presentation The case report describes a 61-year-old female patient who presented with left cheek asymmetry, parotid gland swelling, and significant dilation of Stensen’s duct. The magnetic resonance imaging (MRI) revealed a diffusely reduced parotid gland volume, fibrotic changes and no evidence of inflammatory or malignant lesions. The ductal papilla stenosis was attributed to a previous intervention during which a salivary stone was extracted by incising the papilla. The patient underwent minimally invasive sialendoscopy with ductal dilation to relieve stenosis. Post-procedure, she reported significant symptom relief with reduced swelling. Discussion This case highlights the importance of recognizing and managing ductal papilla stenosis after salivary stone extraction. Sialolithiasis affects more than 1% of the population, mostly the submandibular gland (80-90%). Prognosis is good with early treatment and sialendoscopy reduces recurrence risk. Prevention includes hydration, salivary stimulation and regular follow-up. In this case, minimally invasive techniques restored gland function avoiding surgery. Long-term monitoring remains essential but the patient recovered without complications. Conclusions Chronic parotid gland sialadenitis with ductal stenosis requires precise diagnosis and targeted intervention. This case underscores the efficacy of sialendoscopy and ductal transposition as minimally invasive yet effective solutions for managing chronic sialadenitis, improving gland function and patient quality of life.
