115. The social risk management mechanism of schizophrenia patients from the perspective of public safety

Abstract Background Schizophrenia, as a long-term and complex serious mental illness, is prone to potential public safety hazards when patients return to the community after leaving a fully enclosed medical environment. Previous clinical drug efficacy or single community rehabilitation nursing lacked a systematic governance perspective that deeply integrates public safety with clinical treatment, resulting in lagging social risk management. Therefore, it is necessary to establish a social risk governance mechanism based on the perspective of public safety, explore effective paths to enhance social adaptability, and provide empirical evidence for improving the mental health service system. Methods The study selected 248 community schizophrenia patients under the key control of a certain city’s mental health center and local police station as the subjects. A multidimensional linkage governance model was established and a 12-month graded risk intervention was implemented. Use the Positive and Negative Syndrome Scale (PANSS) to assess clinical symptom improvement, and use the Social Disability Screening Schedule (SDSS) to evaluate social functioning. The data was statistically processed using SPSS 26.0 software, with t-test for quantitative data and chi square test for quantitative data. The changes in risk indicators before and after the implementation of the governance mechanism were compared. Results The comparison results of clinical symptoms and social risk indicators of patients before and after the implementation of governance mechanisms are shown in Table 1. According to Table 1, after implementing the comprehensive management mechanism, patients’ various risk indicators and clinical symptoms showed significant improvement. Among them, the incidence of impulsive aggressive behavior among patients has significantly decreased, and the compliance with regular medication has greatly improved. The total score of PANSS after implementation decreased from 89.45 points to 64.20 points, with a significant difference (p<.05). The rate of incidents involving public safety has sharply decreased from 18.55% to 3.22%, with a statistically significant difference (p<.05). This indicates that the multi-party collaborative governance mechanism effectively blocks the path of pathological symptoms transforming into social risk behaviors by strengthening monitoring and timely medical intervention. Discussion Constructing a social risk management mechanism for schizophrenia patients based on the perspective of public safety can significantly reduce the social harm of patients and improve their clinical prognosis, achieving the dual goals of safeguarding public safety and protecting patient rights. This mechanism can effectively improve the timeliness and accuracy of crisis intervention. Future research directions are suggested to focus on using artificial intelligence and big data technology to establish a dynamic risk warning model, and further explore the legal and ethical boundaries in the process of connecting compulsory healthcare and community correction, in order to promote the digital and legal upgrading of the governance system.
