66: Code ECPR – A Multidisciplinary Approach to Saving Lives

Introduction: ECPR is the integration of VA-ECMO(Some Times VV for long time hypoxia) during cardiac arrest when conventional CPR fails to achieve return of spontaneous circulation (ROSC). “Code ECPR” represents a structured, team-based protocol to rapidly identify candidates and initiate ECMO support in refractory cardiac arrest. Objective: To evaluate the effectiveness and outcomes of a multidisciplinary “Code ECPR” response system in improving survival and neurological recovery following cardiac arrest. Methods: A retrospective review of patients who underwent ECPR between 2022–2025 at Apollo Multispecialty Hospitals Kolkata, Data were analyzed regarding time-to-ECMO initiation, etiology of arrest, survival to discharge, and neurological outcome. Key focus was placed on coordination between perfusionists, intensivists, emergency physicians, and nursing staff during activation of Code ECPR. Conclusion: Implementation of a structured “Code ECPR” system enhances coordination, reduces response time, and improves survival and neurological outcomes in refractory cardiac arrest. Early recognition, team training, and protocolized communication are key elements to ECPR success.
