(O-28) Analysis of erection quality in patients with penile cancer undergoing surgical treatment

Abstract Introduction Malignant neoplasm of the penis is a rare condition around the world, and is closely associated with poor hygiene habits. In approximately 60% of cases, it occurs in individuals under 60 years of age, abruptly impacting the erectile function of men who are, for the most part, sexually active. Despite several organ-sparing treatments, the anatomical disfiguration frequently associated with the diagnosis and treatment of penile cancer negatively impacts quality of life and sexual function, which makes this subject an important point of discussion. Objective To evaluate the impact of surgical treatment on sexual function and satisfaction in patients with penile cancer. Methods This is a prospective study that analyzed patients undergoing partial penectomy in two tertiary hospitals between 2023 and 2025. Patients responded to the Simplified International Index of Erectile Function (IIEF-5), through four items that assess erectile dysfunction (ED) and one item of satisfaction during intercourse. To analyze the results, each alternative was given a score, with ED classified into categories according to the sum of the scores obtained: 1 to 7 characterizing severe dysfunction, 8 to 16 moderate, and 17 to 21 mild. Results During the study period, 28 patients underwent partial penectomy for penile cancer, and 23 patients were evaluated, whose mean age was 60 years, ranging from 43 to 78 years. The penile segment removed varied according to the clinical manifestation and stage of each case. Approximately 78% of the patients presented some degree of ED in the postoperative period, of which 9% had mild dysfunction, 39% had moderate dysfunction and 47% had severe dysfunction. A common complaint among patients with severe ED was a change in their relationship with their partner after the procedure. Conclusions The data presented clearly demonstrate the negative impact of diagnosis and surgical treatment on the sexual function in patients with penile cancer. Financing No conflict.
