Adaptation of the Partners at Care Transitions Measure Into Turkish: A Validity and Reliability Study for Older Adults

This study provides evidence supporting the psychometric adequacy of the Turkish version of the PACT-M. The instrument appears to be a useful tool for assessing older adults' perceptions of care quality and safety during the transition from hospital to home. Its application in clinical practice may help identify patients who require additional support during the discharge and post-discharge process.
