Next-Generation Herbal Medicines for the Management of Osteoarthritis: A Pharmacological and Pharmaceutical Perspective

Abstract: Osteoarthritis (OA) is an age-conditioned and multifactorial joint cartilage loss disease, subchondral bone remodeling, synovitis, and chronic pain. It significantly affects load-bearing joints such as the hips, knees, and spine. Despite intensive use of conventional therapeutic modalities such as non-steroidal anti-inflammatory drugs (NSAIDs), corticosteroids, and surgery, these therapeutic interventions primarily provide symptomatic relief with little disease-modifying activity and are often associated with toxic side effects. This has raised increasing interest in new-generation herbal medicine mechanisms as safer and more natural therapeutic agents for OA management. Herbal medicines from medicinal plants like Curcuma longa, Boswellia serrata, Zingiber officinale, and Withania somnifera were found to exhibit promising anti-inflammatory, antioxidant, and chondroprotective activities, according to preclinical and clinical research studies. These phytochemical compounds regulate pertinent molecular targets such as pro-inflammatory cytokines, matrix metalloproteinases (MMPs), and nuclear factor-kappa B (NF-κB) signaling pathways and thus modulate cartilage metabolism and joint inflammation. Besides this, herbal drugs are multi-targeted in action with low toxicity, i.e., they possess the ability to act both as symptomatic and disease-modifying agents. This review presents a complete pharmacological and pharmaceutical characterization of the emerging new role of next-generation herbal drugs in OA, viz., their mode of action, bioactive phytochemicals, clinical data, and drug design paradigms. By the interfusion of age-old wisdom and current pharmacology, herbal drugs present a new arena in the integrative management of osteoarthritis.
