Ambient Artificial Intelligence (Ambient AI) undergoes clinical trial evaluation for clinical practice applications
Artificial intelligence (AI) is reshaping the medical field. From patients to clinicians, everybody is using AI. It is totally changing medical science, increasing efficiency and accuracy. However, if it’s free, it becomes accessible to everyone. Both patients and clinicians are using AI for various purposes[1]. However, the growing use of AI highlights the necessary to establish clear benchmarks for its use. In this direction, Agha et al illustrated the transparency standards for AI reporting. It provides a guideline for the use of AI, known as the TITAN guideline. It outlines benchmarks for researchers using AI in their studies[2]. Similar to other AI platforms, Ambient AI is one of the most significant platforms. Recently, it has been used in the medicine and health care field. Ambient AI refers to AI systems that are integrated into the environment. It is allowing them to operate in the background without requiring direct human intervention. In health care, Ambient AI is being used to reduce clinicians’ administrative workload. Therefore, Ambient AI reduces the time required for clinical documentation[3-5]. In a clinical setting, a pilot study was conducted to assess the clinician’s experience before and after using Ambient AI. In this study, Stults et al found that both doctors and patients reported that the Ambient AI software was user-friendly and useful[6]. However, many clinicians reported that significant improvements are necessary. In health care, clinical documentation is a significant challenge. It has been noted that clinical note writing takes considerable time from physicians. In this case, the physician feels exhausted. At the same time, the big clinical note writing, parallel to other clinical practices, is one of the causes of dissatisfaction. In this case, enormous documentation may also destabilize the relationship between the physician and the patient. At the same time, it might reduce the quality of patient care. On the other hand, maintaining electronic health records (EHRs) is a very complex job. In this direction, physicians are working outside their regular hours with EHR systems to complete documentation. But EHR is a complex system for doctors. It creates a substantial problems and raises concerns about the sustainability of the physician workforce[3,7,8]. To address these issues, Ambient AI has been developed as an essential tool for doctors. It can listen to patient–clinician interactions and helps automatically generate clinical notes. Therefore, it is reducing the documentation burden in the health care system. Recently, Ambient AI software has been launched to support doctors in clinical documentation using EHRs. The researchers at Abridge AI Inc., USA, developed this AI software. A clinical trial was initiated to understand the efficiency of this AI software in routine patient care. The trial outcomes will be measured by three components: the efficiency of providing clinical notes, physician job-related satisfaction, and the accuracy of medical coding. In this clinical trial, 66 physicians participated from eight different medical speciality fields. The trial was commenced in June 2024. The official trial phase began in October 2024 (clinical trial id: NCT06517082). The Abridge AI system integrates with Epic electronic health records and, following patient consent. In this study, it has been noted that the Ambient AI software can automatically generate clinical notes. There is a dedicated guidebook. Following the guidebook, researchers are implementing the trial. The computer monitor system shows utilization (approximately 65%), time spent on clinical notes, and medical coding accuracy. The system’s coding accuracy has been noted, and it was noted that it aligns with professional coders (about 97%). Here, a preliminary test was conducted with 20 physicians using Ambient AI. It reduced note-taking time by approximately 20 minutes per day[9]. In the clinic or hospital, clinicians are adopting Ambient AI, and its use is increasing now a days[3-5]. At the same time, for Ambient AI, both clinicians and patients reported their experiences and indicated their positive experiences. They reported that Ambient AI can perform better as a clinical record-keeping tool. However, no decrease in time was noted in the medical documentation. At the same time, the advancement of Ambient AI will continue. It is expected that the technology will transform the clinical landscape. However, monitoring and technological improvements are necessary from time to time to achieve the best performance.
