A Case Report of a Patient with Diabetic Foot Ulcer Improved by Integrative Treatment Including Taklisodok-um
Background: Diabetic foot ulcers occur because of a complex interplay between vascular impairment induced by chronic hyperglycemia and peripheral neuropathy, leading to sensory deficits and decreased immune resistance. Delayed treatment can result in severe infection and necrosis, making continuous management crucial for reducing recurrence rates. Although numerous cases of diabetic foot ulcers have been reported, clinical research on Korean medicine approaches remains limited.Case presentation: A 67-year-old male with a history of a right hallux diabetic foot ulcer underwent four reconstructive surgeries and subsequently received integrative treatment including Taklisodok-um. At the time of diagnosis, the diabetic foot ulcer was classified as Wagner Grade 4, indicating partial forefoot gangrene. After reconstructive surgery, the lesion progressively improved during hospitalization and ultimately reached Grade 0, characterized by complete epithelial recovery without ulceration. Taklisodok-um, a traditional Korean medicine with anti-inflammatory and wound-healing properties, appears to play a role in supporting tissue regeneration and promoting wound closure. This improvement was also influenced by natural postoperative healing processes, enhanced peripheral circulation, initiation of insulin therapy, and changes in the dressing method. In addition, the patient developed a neuropathic foot deformity accompanied by gait disturbance and aggravation of bilateral ankle, calf, and lower back pain; however, these symptoms markedly improved after acupuncture and wet cupping therapy.Discussion and conclusion: Integrative treatment including Taklisodok-um demonstrated promising clinical outcomes in diabetic foot ulcer management. Further systematic studies are needed to elucidate the precise roles and mechanisms of action of Taklisodok-um.
