The intersection of osteoporosis and cardiovascular disease in pathogenesis and treatment

Osteoporosis and cardiovascular disease (CVD) are two major public health concerns that share overlapping pathophysiological mechanisms, including chronic inflammation, oxidative stress, and calcium metabolism dysregulation. Emerging evidence suggests that these conditions are not independent but rather interconnected through common risk factors such as aging, hormonal imbalances, and metabolic disorders. Despite this growing recognition, clinical management often remains fragmented. This perspective explores the intricate relationship between osteoporosis and CVD, emphasizing shared mechanisms and potential therapeutic strategies. Pharmacological interventions, including bisphosphonates, statins, and novel agents like RANKL and sclerostin inhibitors, may offer dual benefits for bone and cardiovascular health. However, concerns regarding calcium supplementation and vascular calcification necessitate a cautious approach. Lifestyle modifications, including exercise, dietary optimization, and smoking cessation, play a critical role in mitigating both conditions. A multidisciplinary, integrated management approach is essential for improving patient outcomes. Future research should focus on refining treatment strategies and developing guidelines that address these interconnected diseases comprehensively. There is a pressing need for the development of integrated clinical guidelines that reflect the shared pathophysiology and therapeutic targets of osteoporosis and CVD. Establishing a collaborative framework between specialties could help unify care pathways and optimize long-term outcomes for affected patients.
