Fertility in Polycystic Ovary Syndrome—The Spark and the Fire!
Polycystic ovary syndrome (PCOS) is the commonest endocrine abnormality in reproductive-age women, typically presenting with oligo and/or anovulation, clinical or biochemical hyperandrogenism, and polycystic ovarian morphology (PCOM). Infertility in PCOS is primarily due to anovulation, which is fuelled by insulin resistance (IR), hyperinsulinemia, hyperandrogenism, and obesity. With targeted interventions and a comprehensive workup and treatment, most of these women conceive. Lifestyle modifications remain the cornerstone of managing fertility in these women. Medical interventions include ovulation induction, treating IR, and the use of in vitro fertilization (IVF). Laparoscopic ovarian drilling and the use of bariatric surgery are the surgical options that help with fertility in these women. With accurate diagnosis and a personalized fertility treatment plan, achieving a healthy pregnancy is absolutely possible.
