Pregnancy-associated Breast Cancer: An Obstetrician\'s Dilemma

Pregnancy-associated breast cancer (PABC) is defined as any breast malignancy diagnosed during pregnancy, lactation, or within 1 year of delivery. It is a rare entity and still the most common malignancy in pregnancy, with the most common histopathology being invasive ductal carcinoma. We present a case of a second gravida female with a history of previous cesarean section who presented to us at stage IV breast cancer with spinal metastasis at 24 weeks of gestation. She was given doxorubicin and cyclophosphamide as chemotherapy antenatally. She was terminated via cesarean section at 35 weeks and delivered a healthy baby boy.Postnatally, gemcitabine and cisplatin were given for six cycles as second-line chemotherapy for bony metastasis, followed by palliative radiation and tamoxifen given 18 months later. The placenta was free of metastasis. The baby had normal developmental milestones at the age of 2 years on follow-up.It is emphasized that a multidisciplinary approach is needed for the holistic care of the mother and fetal dyad.
