13 02 Metastasis Base Architecture Gastric Diffuse Signet-Ring Plastica
Linitis plastica represents the most aggressive gastric carcinoma phenotype, characterized by diffuse infiltration,signet-ring cell morphology with massive intracellular mucin accumulation (60-70% cytoplasmic volume), andexceptional metastatic propensity . While accounting for only 7-10% of gastric cancers, LP exhibits disproportionately poor outcomes with median overall survival of 6-8 months and 5-year survival rates below 10%. The metastatic pattern is distinctive: peritoneal carcinomatosis dominates (65-80%), followed by lymphatic spread (75-90%), with substantial subsets developing liver (30-40%) and ovarian metastases (Krukenbergtumors, 40-50% in females) .Current therapeutic approaches fail to address LP’s fundamental ecosystem architecture at each metastaticsite. Each microenvironment imposes distinct selective pressures- peritoneal hypoxia and acidification, hepatichyperoxic stress, lymphatic immune surveillance, ovarian hormone exposure- triggering site-specific organellaradaptations that drive survival and therapeutic resistance. Understanding these adaptations at quantitativeresolution is essential for identifying actionable vulnerabilities.
