Reviving the Unseen: Emergency Resuscitation and Diagnosis of Hypertrophic Cardiomyopathy
Apical hypertrophic obstructive cardiomyopathy (AHOCM) is a rare variant of hypertrophic cardiomyopathy characterized by hypertrophy predominantly localized to the apex of the left ventricle. We present a case of a 21-year-old male who experienced sudden cardiac arrest precipitated by physical exertion. Prompt initiation of cardiopulmonary resuscitation and subsequent management led to successful resuscitation and diagnosis. The patient was found to have severe metabolic acidosis, marked electrocardiographic abnormalities, and echocardiographic evidence of AHOCM. Timely intervention with an implantable cardioverter-defibrillator (ICD) was undertaken, and the patient was discharged after an uneventful recovery period. This case highlights the importance of considering AHOCM as a potential cause of sudden cardiac arrest in young individuals without significant cardiovascular risk factors. Prompt recognition, resuscitation, and appropriate management, including ICD insertion, are essential for optimizing outcomes in these patients. Further research is warranted to elucidate optimal management strategies for this rare cardiac condition.
