Management of Vincristine Extravasation Using Dimethyl Sulfoxide

Extravasation is a chemotherapy complication that limits optimum patient management and requires immediate attention. Hyaluronidase is the standard antidote suggested for use in vinca alkaloid-related extravasation injury as soon as possible. Here, we report a case of extensive cutaneous damage to the volar surface of the arm after receiving a vincristine infusion. Due to hyaluronidase not being available in our country, we used a dispersion of 10% Vitamin E in 99% dimethyl sulfoxide (DMSO), starting 8 days after the accident, which resulted in significant improvement. The lesion had completely improved in the last visit, with only post-inflammatory hyperpigmentation remaining. Due to the radical scavenging properties of DMSO, the use of said dispersion for vincristine extravasation injuries may be a potential option.
