Therapeutic Potential of Flavonoids in the Management of Diabetic Neuropathy Pain: Mechanisms, Clinical Evidence, and Challenges

Introduction: Diabetic neuropathy, a serious complication of diabetes mellitus, impairs quality of life through chronic pain and sensory deficits. Current drugs like gabapentin, pregabalin, and opioids offer limited relief and cause notable side effects. Flavonoids— polyphenolic compounds in fruits, vegetables, and herbs—have attracted attention for their antioxidant, anti-inflammatory, and neuroprotective properties. This review examines their therapeutic potential in diabetic neuropathy pain, focusing on mechanisms, preclinical and clinical evidence, and application challenges. Methodology: A comprehensive literature review assessed preclinical studies, clinical trials, and mechanistic data on flavonoids, including quercetin, curcumin, hesperidin, and rutin, evaluating their efficacy, safety, and limitations. Results: Flavonoids alleviate diabetic neuropathy pain by modulating oxidative stress, inflammation, and ion channel activity. Preclinical findings show reduced pain sensitivity, improved nerve conduction velocity, and neuroprotection. Clinical studies report promising benefits, with compounds like quercetin and curcumin reducing discomfort and improving quality of life. However, poor bioavailability, lack of standardized dosing, and variability in patient response hinder widespread use. Discussion: The pharmacological profile of flavonoids supports their role in managing neuropathic pain, but translation into clinical practice requires overcoming bioavailability and dosing issues. Strategies such as nano formulations, combination therapies, and rigorous clinical trials are needed to validate safety and effectiveness. Conclusion: Flavonoids are promising natural candidates for the management of diabetic neuropathy pain, targeting multiple pathophysiological mechanisms and demonstrating a favorable safety profile. Further research should optimize formulations, standardize dosing, and confirm long-term efficacy to fully harness their therapeutic potential.
