Alar retraction is an aesthetically and functionally challenging nasal deformity that can significantly affect nasal airflow and patient satisfaction. This case report describes a 24-year-old woman presenting with bilateral alar retraction and nasal obstruction who desired nasal tip augmentation.Surgical correction was achieved through open rhinoplasty using a septal extension graft, onlay graft, cut-and-overlay technique, alar base reduction, and septoturbinoplasty. The procedure resulted in aesthetic improvement and resolution of functional symptoms. This case highlights the importance of comprehensive analysis and individualized surgical planning in addressing alar retraction.
