148 Prospective Evaluation of Semaglutide (Wegovy) in a Tier 3 NHS Obesity Service: Six-Month Real-World Outcomes from a District General Hospital

Abstract Aim This study aimed to evaluate the efficacy of Semaglutide (Wegovy), a GLP-1 receptor agonist, as part of a multidisciplinary Tier 3 obesity management service over a six-month period in a district general hospital setting. Method We conducted a prospective evaluation of 18 patients who commenced on Wegovy in April 2025. Patients were followed up at defined intervals over six months and were supported by behavioural and talking therapies. Primary outcomes included weight change and treatment adherence. Secondary outcomes included tolerability, service engagement, Generalized Anxiety Disorder 7-item scale (GAD-7), Patient Health Questionnaire 9 (PHQ-9) scores, and observed impact on obesity-related comorbidities where data were available. Results Overall, 89% of the patients in our study were female and 11% were male. All participants adhered to the prescribed Wegovy treatment regimen, with only one patient reporting nausea as an adverse effect following dose escalation to 1 mg. The mean BMI at baseline was 50.72, and all patients had multiple, well-controlled comorbidities. At the 3-month follow-up, patient adherence remained high. During follow-up consultations, GAD-7 and PHQ-9 questionnaires were administered, demonstrating an average reduction of 2–3 points in 80% of the participants. Additionally, a mean weight loss of 2 kg was observed across the study population. Conclusions Despite a modest cohort size, initial findings indicate encouraging trends in weight reduction, good treatment persistence, and favourable tolerability. These real-world insights complement existing trial evidence and highlight the potential and challenges of implementing advanced obesity pharmacotherapy in local NHS services.
