287 Management and Predictors of Outcome in Oligometastatic Pancreatic Ductal Adenocarcinoma: Protocol for a European Snapshot Study

Abstract Aim It is not clear if local consolidative treatment (LCT) for oligometastatic pancreatic ductal adenocarcinoma (PDAC) is favourable, although large, specialised centres are increasingly undertaking this in select individuals. This study aims to determine the current real-world European selection criteria for LCT in this group and determine predictive features where this may be beneficial compared to standard of care. Method The OPANC Registry will collect two cohorts: 1) all patients undergoing synchronous LCT for oligometastatic PDAC in European centres between 2019 and 2024; 2) all patients with synchronous metastatic PDAC at presentation in 2022. Demographic, functional, treatment, pathological and outcome data will be collected, focusing on radiological and biological response to therapy. Propensity score matching and uni- and multivariable logistic regression models will be used to determine factors which are associated with receiving LCT and those which predict outcome. Results The study protocol has ethical approval from the Technical University of Munich and the OPANC Registry is hosted by Amsterdam UMC on the Castor Database. The study will open to recruitment in Summer 2025. Centres in Germany, Italy, the Netherlands, Turkey, Spain, Romania, Sweden, Finland and the UK have registered interest. Conclusions Determining which, if any, patients with metastatic PDAC should undergo LCT is controversial. This study will provide the largest real-world indication of selection and predictors of outcomes for LCT in oligometastatic PDAC in a European setting. Hopefully, this study will provide further evidence to aid the development of European consensus guidelines for this patient group.
