379 Improving Documentation of Severity Scores in Acute Pancreatitis: A Two-Cycle Audit Against AUGIS Guidelines

Abstract Background Severity scoring in acute pancreatitis is vital for early risk stratification, guiding escalation decisions and improving patient outcomes. The Association of Upper Gastrointestinal Surgeons (AUGIS), alongside the British Society of Gastroenterology, recommends that all patients with acute pancreatitis have a severity score documented within 48 hours of admission using a validated tool. Aim To assess adherence to AUGIS-endorsed guidelines on severity score documentation in acute pancreatitis and evaluate the impact of local quality improvement interventions. Method A retrospective audit was carried out across two cycles:Cycle 1 (1 Jan – 28 Feb 2025): 24 patients were identified. Only 4 (17%) had severity scores documented.Intervention: Local initiatives included FY/SHO teaching, staff posters summarising guidelines, and an electronic documentation template.Cycle 2 (1 Jun – 12 Jul 2025): 23 patients were reviewed. Seven (30%) had documented severity scores — all completed within the first 24 hours. One additional case was incomplete due to pending labs.Scoring systems used included Glasgow-Imrie, Modified Glasgow, and BISAP, based on clinician preference and data availability. Conclusions Following low baseline compliance, structured educational and system-level interventions improved both the rate and timeliness of severity score documentation. Embedding these practices into routine care supports adherence to AUGIS guidelines and enhances early risk stratification in acute pancreatitis.
