Corneal lamellar patch graft: A remedy for hypotony maculopathy and leaking bleb
Trabeculectomy with antifibrotic agents such as Mitomycin C has merits but comes with the risk of bleb-related complications such as bleb leaks and hypotony maculopathy. This can be managed with several techniques, including conservative and surgical approaches. Corneal lamellar patch graft offers multiple advantages over other surgical techniques such as scleral patch graft and amniotic membrane graft. We report a patient who presented with a leaking thin cystic avascular bleb, intraocular pressure of 3 mmHg, and hypotony maculopathy. The patient was initially managed conservatively and later managed surgically with bleb excision by bleborhexis and clear corneal lamellar patch graft. Timely recognition and intervention thus help in preventing sight-threatening complications.
