Late-onset acute psychosis due to right thalamic infarct
Introduction Several studies have established the thalamus as a critical node in the pathways relevant to the development of psychotic symptoms and a candidate biomarker for psychosis. Acute-onset psychosis can occur after structural lesions affecting specific thalamic nuclei. Case report We present a case of a sixty-seven-year-old woman who developed new-onset psychosis following a medial thalamic stroke. The lack of signs and symptoms classically associated with a stroke posed a diagnostic challenge. Treatment with antipsychotic medication provided symptomatic relief. Conclusion This case underscores the thalamus’s role in psychosis and the need to exclude secondary causes in atypical presentations of psychotic symptoms.
