VirtuGuide: Precision Correction of Hallux Valgus Deformity Utilizing Preoperative CT and Radiographic Analysis

Research Type: Level 4 – Case series Introduction/Purpose: Hallux valgus deformity correction often necessitates precise three-dimensional planning to achieve optimal outcomes. This study evaluates the accuracy of VirtuGuide, a novel system that utilizes preoperative CT and plain radiographic data to create a deformity-specific surgical plan. This system guides correction at the first metatarsal-cuneiform joint, focusing on intermetatarsal angle (IMA) and frontal plane angle (FPA) correction through valgus reduction and derotation of the first metatarsal. Methods: Six patients undergoing bunion correction from 2 surgeons with VirtuGuide preoperative analysis are included in this retrospective review. Preoperative CT and radiographs were used to determine the IMA and FPA, establish correction goals, and generate a surgical plan with specific target correction values. Postoperative weight-bearing radiographs at 6 weeks were obtained and measured. The postoperative IMA and FPA values were compared with the preoperative target correction values. The absolute difference between the target and postoperative values was calculated to assess accuracy. Descriptive statistics were used to summarize the data. Fixation varied between the operative surgeons as the deformity correction system is fixation agnostic. Results: In all six cases, the postoperative IMA and FPA values closely approximated the preoperative target correction values. Variation from the pre-operative plan was less than 1 degree from target correction value for both measures in all cases. Conclusion: VirtuGuide demonstrates excellent accuracy in correcting IMA and frontal plane deformity in hallux valgus correction. The system's ability to translate preoperative imaging data into precise surgical plans facilitates reliable and consistent deformity correction.
