The Diagnosis and Management of Polycystic Ovary Syndrome in Bangladesh: Position Statement of Bangladesh Endocrine Society
A bstract Background: Polycystic ovary syndrome (PCOS) is a widely prevalent complex endocrine disorder affecting women of reproductive age. It has been associated with various complications, including reproductive, metabolic, dermatological, and psychological challenges. Despite its growing prevalence in Bangladesh, the condition remains underrecognized and suboptimally managed due to a lack of locally developed, evidence-based clinical protocols. Given the distinct metabolic phenotype observed in the Bangladeshi population compared to Western cohorts, there is an urgent and unmet need for comprehensive, culturally appropriate management guidelines tailored specifically to this unique patient group. Materials and Methods: The Bangladesh Endocrine Society (BES) PCOS Taskforce established a 20-member committee to synthesize this position statement for local management. Recommendations were rigorously developed from international PCOS guidelines, randomized controlled trials, national observational studies, and expert opinion. Through a series of consultative meetings, the committee reviewed and refined the recommendations until achieving unanimous agreement on the proposed guidelines. Results: The resulting position statement provides comprehensive, locally tailored guidelines for managing PCOS. This includes sections on patient evaluation, management (lifestyle and drug treatment), addressing complications, and specific guidance on follow-up care relevant to the Bangladeshi clinical setting. Conclusion: This BES Position Statement provides the first evidence-based, locally tailored protocols for managing PCOS in Bangladesh. This initiative is expected to standardize assessment and significantly improve treatment efficacy across all levels of care.
