Loss of substance of the distal extremities of the long fingers : Interest and viability of the atasoy-type cover flap, prospective study of 15 cases
Sub-amputation of the finger pulp is an important aspect of hand surgery. After several procedures using remote flaps, such as toe pulp transfer, the current trend is towards local and regional pedicled flaps, each with its own indications, taking into account the pulp involved, the size of the defect and surgical schools. Our prospective study concerns 15 cases, aged between 30 and 45, admitted for management of loss of cutaneous substance of the distal phalanx of a long finger, operated on with an ATASOY-type VY advancement flap and followed up in our department, from February 2021 to April 2023. One case in our series presented with the onset of peripheral flap necrosis on day 2. Three other patients presented with sero purulent discharge at the 10th-day follow-up visit, which resolved spontaneously. One patient was seen on day 15, presenting a necrotic finger. He was re-hospitalized for radical treatment. The rest of our patients progressed well, with good flap healing. The ATASOY flap is an increasingly popular local flap for distal tissue loss, providing high-quality trophicity and good coverage, both mechanically and tactilely
