Evaluation of the Use of Hem-O-Lok® Clips in Appendicular Stump Closure during Laparoscopic Appendectomy
Background: Acute appendicitis remains the most popular surgical emergency, affecting approximately 8.6% of males and 6.7% of females over their lifetime. The study aimed to evaluate the hem-o-lok clip’s feasibility, safety, operative time, and liability to cause post-operative problems when used for appendicular stump closure and contrast the results with the usage of hand-made intracorporeal sutures for stump closure.Patients and Methods: This randomized controlled clinical trial was carried out from July 2023 to July 2024. The study involved 80 patients diagnosed with acute uncomplicated appendicitis. The patients were randomly allocated into two groups using the closed-envelope method. Group A underwent laparoscopic appendectomy (LA) with Closure of the appendicular stump with Hem-o-lok® clip, and group B underwent LA with closure of the appendicular stump using intracorporeal suturing. Results: Each group consisted of 40 patients. Group A's operating time (38.1 minutes) was substantially less than Group B's (52.8 minutes). Group A's appendicular stump closure time (5.9 minutes) was noticeably less than Group B's (10.4 minutes). Regarding intraoperative complications, they weren’t observed in each group. Postoperative stay in the hospital was the same in both groups. Additionally, no cases were rehospitalized after discharge. Therefore, there was no difference of statistical significance in the two groups' incidences of complications. Conclusions: Because of its simplicity, ease of use, and low learning curve, the Hem-o-lok clip is a secure and useful choice for securing the appendiceal base. It also offers a quicker operating time.
