Transurethral Diode Laser Ablation of Prostate Vs Bipolar Transurethral Vaporization of Prostate, A Randomized Controlled Study

Background: One of the common treatments for lower urinary tract symptoms (LUTS) caused by benign prostate hyperplasia (BPH), particularly in men who have a tendency to bleed, is vaporization of the prostate. Objective: to compare the short-term safety, effectiveness, and cost of bipolar vaporisation with transurethral diode-laser ablation of the prostate utilizing BiolitecR's Xcavator fiber throughout a 6-month post-operative follow-up period.Patients and Methods: 60 individuals with prostate sizes ranging from 30 to 80 grammes were involved in this randomized control trial. Group a received transurethral diode laser ablation (DLA), whereas group B (control group) received bipolar transurethral vaporization (BVP). They were split into two equal groups. Data from intraoperative, postoperative, one-month, and six-month follow-ups.Results: Both procedures showed that they are safe and efficient on patients complaining from LUTS secondary to BPH especially high-risk patients taking anticoagulants, and both of them had great results concerning signs and symptoms of BPH (Q-max increase, IPSS decrease and post voiding residual decrease). Our results showed that both techniques have no risk of intraoperative bleeding, no risk of blood transfusion and no risk of TUR syndrome. However, results showed that diode laser transurethral prostate ablation is a faster technique with mean of operation time of 22 minutes compared to 49 minutes in the bipolar prostate vaporization technique. Conclusion: Both methods are thought to be safe, effective, and have comparable results. They also have good hemostatic profiles, and no significant adverse effects have occurred during either surgery. Although laser treatment is quicker than bipolar, it is more costly, thus it should be saved for bigger prostates and other procedures like enucleation.
