Abstract Esflurbiprofen patch is a topical nonsteroidal anti-inflammatory drug (NSAID) that may increase the international normalized ratio (INR) and the risk of bleeding in patients taking warfarin. This article presents a case study of a 71-year-old Thai male with atrial fibrillation who was maintained on warfarin at a dose of 17.5 mg/week, achieving a stable INR within the therapeutic range of 2.00 - 3.00. Due to an unexplained decrease in INR to 1.70, his warfarin dose was increased to 21 mg/week. Two to three days before his follow-up appointment, the patient self-administered an esflurbiprofen patch 40 mg/day in addition to warfarin. Consequently, his INR increased markedly to 6.71, although no abnormal bleeding was observed. Warfarin was temporarily discontinued for four days, and upon resuming a dose of 17.5 mg/week while discontinuing the esflurbiprofen patch, the patient’s INR returned to 2.00 after two weeks. This case highlights the potential risk of increased INR and bleeding associated with esflurbiprofen patch use in patients on warfarin. Healthcare professionals should be aware of this interaction and closely monitor INR levels in such patients. Keywords: esflurbiprofen patch; warfarin; INR; drug interaction; bleeding
