Ileal conduit complications. Management strategies
• Importance: As the ileal conduit is the most widely used form of urinary diversion, addressing its complications adequately is critical to ensuring patient safety. • Feasibility: Robotic management of ureteral-ileal strictures is both practical and effective, offering favorable outcomes. • Parastomal Hernia is the most frequently encountered issue, emphasizing its significance in postoperative care. The ileal conduit is the most commonly performed urinary diversion following radical cystectomy, yet it is associated with a high complication rate, reaching up to 77 %. Complications are typically categorized by anatomical location, including the uretero-ileal anastomosis, the stoma, and the ileal segment. This article showcases the robotic management of ileal conduit complications across diverse clinical scenarios, integrating insights from a literature review with detailed video presentations of complex clinical cases. The cases included uretero-ileal anastomosis strictures, ileal segment complications, and stoma-related complications. Surgical steps with technical tips are described for each case. Robotic-assisted uretero-ileal anastomosis reconstruction was successfully performed in two cases. A case of parastomal hernia repair included mesh placement; however, postoperative bowel obstruction required laparoscopic exploration, bowel resection, and stoma relocation. Robotic-assisted surgery offers precise and effective management for ileal conduit complications. However, these complex procedures should be performed in centers with expertise to optimize patient outcomes.
