Targeted Immunosuppressive Regimens with Reduced Nephrotoxicity: A Systematic Literature Review

Immunosuppressive regimens are critical for preventing graft rejection in kidney transplantation, but their nephrotoxic side effects often compromise long-term renal outcomes. Although existing protocols successfully inhibit immune reactions, the related toxicity continues to pose a major clinical obstacle, requiring precise approaches to achieve an optimal balance between effectiveness and safety. This systematic literature review examines the principles and strategies of immunosuppression in kidney transplantation, evaluates the toxic effects and management of immunosuppressive drugs, and explores future developments in nephrotoxicity-sparing therapies. A thorough review of published research was performed to compile evidence on adjusted treatment protocols that reduce kidney harm while preserving sufficient immunosuppressive effects. The results indicate calcineurin inhibitor minimization, mTOR inhibitor-based regimens, and new biologic agents hold potential for decreasing nephrotoxicity, but their long-term effectiveness and safety need additional verification. Clinical approaches including therapeutic drug monitoring and individualized dosing also show potential to reduce adverse effects. Novel therapeutic strategies, such as costimulation blockade and methods to induce tolerance, stand as hopeful directions for forthcoming investigations. Notwithstanding these advancements, difficulties remain in applying experimental results to clinical settings, especially in achieving a balance between immunosuppressive strength and kidney safeguarding. Our review underscores the need for continued innovation in drug development and tailored therapeutic strategies to improve outcomes in kidney transplant recipients. Combining mechanistic insights with clinical evidence could lead to safer and more effective immunosuppressive therapies in the future.
