MBT vs Damon Bracket Systems : A Comprehensive Clinical and Biomechanical Comparison

Background: The MBT and Damon bracket systems are two widely utilized fixed orthodontic appliance systems with differing philosophies in biomechanics, torque expression, and treatment strategies. Over the past two decades, they have attracted significant clinical and academic interest due to claims of improved efficiency, reduced treatment time, and enhanced patient comfort. Objective: To evaluate the MBT and Damon bracket systems across multiple dimensions including biomechanical efficiency, treatment duration, arch form development, long-term stability, costeffectiveness, and evidence-based performance. Methods: A structured literature review was conducted using peer-reviewed articles, randomized controlled trials (RCTs), systematic reviews, and cohort studies published in leading orthodontic journals. Comparative analyses were performed under domains relevant to orthodontic practice, ensuring both short-term and long-term perspectives were included. Results: Both systems demonstrated similar treatment outcomes in terms of occlusal finishing, long-term stability, and incisor alignment. Damon exhibited advantages in initial patient comfort, pain control and chair-time efficiency, while MBT offered better control in torque expression and anchorage in extraction protocols. Cost benefit analysis favoured MBT in resource limited settings, whereas Damon was more popular in high income practices emphasizing reduced chair time. Conclusion: Both bracket systems are clinically effective when applied appropriately. Treatment outcomes are influenced more by oral hygiene, diagnosis, treatment planning, and clinician expertise than by bracket choice.
