Erythropoiesis-Stimulating Agents (ESAs): Their Role in Iron Deficiency Anemia

Abstract Background & Rationale: Erythropoiesis-Stimulating Agents (ESAs) are cornerstone therapies for anemia in chronic diseases, particularly in chronic kidney disease (CKD) and cancer-related anemia. Their role in iron deficiency anemia (IDA) is nuanced, as iron availability critically determines efficacy and safety outcomes. Objective: To review and synthesize the pharmacological basis, clinical applications, evidence from randomized trials, safety monitoring, and emerging research directions on ESAs in the context of iron deficiency anemia. Methods: Literature review of major randomized controlled trials, meta-analyses, and updated guidelines (2015–2025) on ESA use in CKD, oncology, and inflammatory anemias, emphasizing iron metabolism, functional deficiency, and therapy optimization. Results & Key Insights: ESAs effectively stimulate erythropoiesis but require adequate iron for maximal response. Intravenous (IV) iron supplementation enhances ESA efficacy and reduces dosing requirements. Key studies (CHOIR, TREAT, DRIVE, FIND-CKD) define safety and hemoglobin targets. Novel agents such as HIF-prolyl hydroxylase inhibitors (HIF-PHIs) offer future alternatives with dual erythropoietic and iron-regulatory benefits. Conclusion: ESAs, when used judiciously with iron supplementation, significantly improve hemoglobin recovery and reduce transfusion needs in complex anemia states. Future practice will integrate biomarkers, genetic predictors, and HIF-based agents for personalized anemia care.
