Impact of educational expansion on trends of limitations in daily activities in Germany

Abstract Background Education is a well-known determinant of health outcomes, yet the impact of increasing educational attainment over time on health trends remains underexplored. This study investigates the role of educational expansion as a mediator in shaping trends in daily activity limitations, assessed via the Global Activity Limitation Indicator (GALI). Methods Using data from the ‘German Health Update’ (GEDA) spanning 2008 to 2024, we applied logistic regression to analyze trends and calculate predicted probabilities for daily activity limitations. Educational disparities were evaluated through the Slope and Relative Index of Inequality. Causal mediation analyses were employed to assess how educational expansion influenced these trends. Results Findings reveal significant educational gradients in daily activity limitations, with disparities growing most notably among men and women aged 40 to 64 years over the study period. Age-specific patterns emerged: older adults showed substantial health improvements, whereas younger adults experienced a notable rise in limitations. Educational expansion played a significant and beneficial mediating role, tempering the increase in limitations among younger groups and enhancing health gains among older groups. Conclusions The expansion of education contributes positively to health outcomes, underscoring the value of continued educational policy advancements. However, the worsening health trends among younger individuals warrant further investigation into underlying causes. Addressing the widening educational disparities, especially in middle-aged groups, is essential for reducing health inequities. Factors associated with societal changes such as the ‘digital divide’ may be critical in understanding and tackling these trends. Key messages • Educational expansion appears to have a positive impact on the temporal development of health, reinforcing the need for continued educational reforms. • Growing health inequities, especially in middle-aged groups, highlight the need to tackle widening educational gradients and underlying socio-political factors to promote health equity.
