Impact of the Aptar Digital Health respiratory disease management platform on adult asthma

Abstract Background United States (US) asthma controller medication adherence (CMA) is 22.2% [1]. Digital therapeutics (DTx) may overcome low CMA. Aptar Digital Health (ADH) respiratory disease management platform (RDMP), a DTx, was used to determine its impact on pressurized metered dose inhaler (pMDI) CMA and rescue medication use (RMU). ADH RDMP includes: 1) HeroTracker connected medical devices track CMA and RMU; 2) BreatheSmart® mobile app provides medication reminders and educational content. Methods ADH RDMP was utilized as part of a larger 18-month study conducted at US outpatient clinics to determine its impact on CMA and RMU. 86 patients used pMDI inhaled corticosteroids (ICS) +/- long-acting beta-agonists and had greater than or equal to 1 data entry each of 18 study months. Primary objective (PO): quantify impact of ADH RDMP on mean monthly (MM) RMU between study start (T0) and end (T1) (18-months). Secondary objectives (SO): 1) evaluate change in MM CMA decline between T0 and T1; 2) Compare T1 CMA to US CMA. Results PO: MM RMU declined 17.3 +/- 35.3 to 7.6 +/- 20.8, a 56.2% reduction (95% [CI -97.1% to -15.4%], p-value .008). SO: 1) MM CMA declined 77.4% +/- 24.6% to 62.1% +/- 33.3%, a 15.3% reduction (95% CI [-28.1% to -11.4%], p-value <.001). 2) T1 CMA, 62.1%, was 39.9% higher than 22.2% US CMA [1]. Conclusions ADH RDMP improved CMA and reduced RMU. T1 CMA, 62.1%, was well above 22.2% US CMA [1]. Data from ADH RDMP studies will be utilized to request global health technology assessments and respiratory societies to include DTx in adult asthma guidelines to improve CMA and to decrease RMU. Global burden of medication non-adherence in chronic obstructive pulmonary disease (COPD) and asthma: a narrative review of the clinical and economic case for smart inhalers. J Thorac Dis. 2021;13(6):3846-3864. Key messages • Use of ADH RDMP significantly improved CMA among adult asthma patients. MM CMA was 62.1% at end of the study, which is substantially higher than 22.2% CMA observed in general US asthma population. • ADH RDMP led to a notable decrease in RMU. MM RMU declined by 56.2% over 18-month study period, indicating that patients needed fewer rescue medications when using platform.
