Serums biomarkers combination of diagnosis for acute appendicitis in childrens

Aim of the study: In children, traditional biomarkers for acute appendicitis, such as white blood cell count (WBC) and C-reactive protein (CRP), remain widely used. Other inflammatory markers, such as calprotectin, procalcitonin, and inflammatory cytokines (IL-6 and TNF-α), are being investigated for their role in the diagnosis and prognosis of severity, especially in complicated appendicitis, and have demonstrated complete separation between appendicitis and control groups. Since acute appendicitis in children can be treated differently, we investigated the combined diagnostic model of interleukin-6, Procalcitonin, and Ferritin in children with acute uncomplicated appendicitis, acute complicated appendicitis, and a control group. Materials and Methods: In this prospective single-center cohort study, IL-6, Procalcitonin, and Ferritin in serum were assayed preoperatively. Children aged 18 years or younger were divided into three groups: AcA, AuA, and Ctr. The predictive values of biomarkers were evaluated using receiver operating characteristic curves. Routine image and other laboratory diagnostics. Results: A total of 152 children from five years were enrolled, including AcA (n = 54), AuA (n = 40), and Ctr (n = 58). The combined diagnostic model was established and showed that the combined diagnostic model AA vs Ctr reached a sensitivity of 90.3 %, a specificity of 90.8 % and an area under the curve of 0.86 (p<0.001). The combined diagnostic model AcA vs AuA reached a sensitivity of 60 %, a specificity of 70 %. Conclusion: The combined diagnostic model of serum predictors at the emergency department may provide a new approach for differentiating between AcA, AuA, and other causes of abdominal pain.
