MON-733 Clinical Adiposity Based Chronic Disease Management, experiences from the clinic post COVID

Abstract Disclosure: T. Htet: None. V. Preda: None. The incidence of obesity has been increasing globally, with WHO data in 2022 reporting 1 in 8 people living with obesity. Obesity is defined as Body Mass Index (BMI) of 30kg/m2 and higher. It is a complex condition to manage, associated with metabolic complications such as insulin resistance, type 2 diabetes (T2DM), hypertension, dyslipidemia and metabolic-associated fatty liver disease (MAFLD) and mechanical complications such as obstructive sleep apnea (OSA) and osteoarthritis. Glucagon-like peptide-1 Receptor Agonists (GLP-1RA) were initially used for management of T2DM. However, newer generations of GLP-1 RAs have been commonly used for obesity management in the past decade. Despite the widespread use of various type of GLP-1 RAs for weight management, there is limited data on their use and effect on obesity in real world practice. Access varies across the world. We review the data from our clinic post COVID return to clinical care. This is a retrospective study reviewing the use and outcomes of the evolving short and long acting GLP-1 RAs in new patients with overweight and obesity attending in an Australian endocrine clinic with multidisciplinary care from 1st of January 2022 to 31st of December 2023. The follow up data up to 31st December 2024 was included. Patients who did not attend for follow up or with incomplete data were excluded. The majority of patients attending our clinic were women with a ratio of 4:1 (female to male). There was a large range in age for whom this condition was significant enough to seek specialist care. The age ranged from 21 and 80 years old. BMI ranged from 27.5-59kg/m2. The follow up duration ranged from 2 months to 25 months with mean duration of 12.5 months. The number of follow up appointments ranged from 2 to 8 with mean follow up appointments of 4.5. Approximately one third of patients (30%) attended the dietitian appointments with less utilisation of exercise physiology and psychology services. Patients who involved expert dietitian care combined with specialist endocrinology care lost more weight than their counterparts. Over one third of patients were managed with GLP-1RA therapy and over a half of patients were treated with combined use of GLP-1RA and metformin. Patients on combination therapy on average lost more weight. Half of the patients had preexisting dyslipidaemia and dysglycaemia or insulin resistance and 30% had hypertension. At least 20% had OSA and or MAFLD. Musculoskeletal symptoms were reported in at least 10% of patients. Holistic approaches to obesity are needed as one in eight adults are affected by this chronic condition. We discuss the real world clinical challenges of obesity management and clinical care follow up. This includes the challenges of clinical access to allied health care. Moreover as we holistically approach weight loss we discuss pharmacotherapy adjuncts and challenges in access, supply chain and product limitations. Presentation: Monday, July 14, 2025
