Sustained Glycaemic Improvement After Duodenal Mucosal Resurfacing in Type 2 Diabetes
Refractory type 2 diabetes mellitus (T2DM) remains a significant therapeutic challenge, as many patients continue to exhibit inadequate glycemic control despite optimal pharmacologic management. Duodenal mucosal resurfacing (DMR) is a novel endoscopic technique designed to rejuvenate the duodenal lining and enhance insulin sensitivity, leading to better glycaemic regulation. This case illustrates a 54-year-old male with a 12-year history of T2DM who remained poorly controlled despite multiple oral agents and high-dose insulin. He underwent DMR resulting in significant improvements in glycemic parameters. The patient’s HbA1c decreased from 10.2% to 7.1% over a three-month period, with concomitant reductions in fasting glucose levels and insulin requirements. This case demonstrates that DMR may serve as a safe and beneficial adjunctive approach for individuals with treatment-resistant type 2 diabetes when conventional management proves insufficient.
