Personal anxiety among the population aged 25-64 in Russia/Mortality and risk of myocardial infarction, stroke: gender differences, (WHO MONICA-Psychosocial program)

The aim of the study was to determine the effect of a high level of personal anxiety on differences in the risk of developing myocardial infarction and stroke in men and women in an open population aged 25-64 years in Novosibirsk. Materials and methods: as part of the III screening of the WHO MONICA-psychosocial program, a random representative sample of the 25-64-year-old population of Novosibirsk was examined in 1994 (men n=657, women n=870). The program included: registration of socio-demographic data, determination of personal anxiety (LT). Over a 16-year period, new cases of myocardial infarction (MI) and stroke accounted for 15 and 35 cases in women; 30 and 22 cases in men, respectively. The statistical analysis was performed using the software package SPSS version 11.5. Criterion χ2 was used, and the Cox–proportional regression model (Cox-regression) was used to assess the risk of development (HR). Reliability in all types of analysis was assumed at a significance level of p<0.05. Results. In the open population aged 25-64 years, VUT was detected in 59.9% of women and 50.9% of men. In Cox's univariate model, after 16 years, HR MI in people with IUT was higher among women than among men (HR =4.19 and HR =3.7, respectively), and stroke, on the contrary, was higher in men than in women (HR =4.43 and HR =3.5, respectively). In the multifactorial Cox model, individuals aged 55-64 years retained the influence of VUT on HR MI, which was higher in women than in men (HR =5.95, HR = 3.56, respectively). The highest risk of stroke in the presence of IUT was in divorced (HR =5,017) and widowed men (HR =3,848) aged 55-64 years (HR =5.8). No such patterns have been identified among women.
