Models of care for people living with multimorbidity in primary care settings: a scoping review protocol of Primary Health Care-oriented approaches
This document outlines the protocol for a scoping review aimed at identifying, describing, and analysing models, programmes, and care elements designed for people living with multimorbidity in primary care settings. The review follows the Joanna Briggs Institute methodology and will be reported according to the PRISMA-ScR extension. The review focuses on assessing the extent to which the identified models incorporate key components of Primary Health Care-oriented approaches, as defined by the World Health Organization. It also aims to characterise their theoretical foundations and core structural components using the multimorbidity care model framework proposed by Stokes et al. (2017). This review seeks to map existing evidence, describe the diversity of current models of care, and identify knowledge gaps to inform future research.
