Empowering and Equipping Europeans with Health Information for Active, Personal Health Management and Adherence to Treatment

The presentation outlines the goals, structure, and progress of the Gravitate-Health project, coordinated by Professor Anne Moen. This EU-funded initiative aims to enhance citizens' access to trusted digital health information—particularly electronic Product Information (ePI)—to improve understanding, adherence to treatment, and overall health outcomes. Gravitate-Health is a public-private partnership involving 45 partners across Europe and the USA, running from November 2020 to June 2026. The project focuses on developing a federated open-source platform with G-lens® services, integrating ePI with personal health data using international standards like HL7 FHIR and IDMP. It employs agile methodologies and conducts pilot studies in eight countries, using realistic personas and scenarios to test usability and impact. The CAPABLE platform is highlighted as a key tool offering personalized medication lists, health education materials, and two-way communication with healthcare providers. Gravitate-Health promotes cross-border interoperability, multilingual access, and risk minimization strategies, contributing to safer medication use and better patient engagement. The project emphasizes innovation through SMART principles—Safe, Meaningful, Accessible, Reliable, and Trusted—and continues to engage stakeholders through hackathons, research collaborations, and ecosystem outreach.
