'Tooth Preparation Burs'- Recent Advancements: A Review.

ABSTRACT Digital processes, an increasing focus on minimally invasive dentistry, and material science have all contributed to recent advancements in tooth-preparation burs. When working with high-strength ceramics (like zirconia) and contemporary restorative materials, new abrasive technologies—such as nano-diamond and chemical-vapor-deposited (CVD) diamond coatings, polycrystalline diamond (PCD) tooling, and enhanced sintered/embedded-diamond manufacturing—offer significantly higher abrasion resistance, longer tool life, and improved surface finish. At the same time, chip evacuation and cutting efficiency are improved while chipping and subsurface damage are decreased by optimized micro-geometry and flute designs that are suited to certain substrates (green vs. sintered ceramics, PMMA, composites). Milling-specific tool families (nano-diamond-coated and PCD bits) have been created through integration with CAD/CAM ecosystems, which reduce post-processing requirements and enhance dimensional stability during production runs. Though at slower bulk-removal rates, energy-based and minimally invasive methods, particularly ultrasonic/CVD diamond tips, offer conservative cutting with less vibration and patient discomfort. Just-in-time replacement, predictive maintenance, and standardized replacement methods to maintain marginal and internal fit are promised by emerging production processes (additive and bespoke tooling) and IoT-enabled tool-wear monitoring. Future directions include material-specific tooling libraries, sustainability considerations (validated single-use vs. sterilizable multi-use), rigorous clinical validation of bench findings, and broader clinical usage of PCD/CVD technology. Standardized testing procedures, harmonization of machine-tool-material parameters, and high-quality clinical outcome data that connects tool wear and selection to restoration longevity and patient-centered outcomes are necessary for implementing these advancements into regular practice. Keywords: Tooth preparation Burs; Nano-diamond; CVD diamond; Polycrystalline diamond (PCD); CAD/CAM milling; Ultrasonic diamond tips.
