In vitro activity of Fosfomycin among uropathogens in a tertiary health care center

ABSTRACT Introduction: The increasing drug resistance among the uropathogens and a lack of effective oral antibiotic have limited the therapeutic options available for urinary tract infections (UTIs). This shortage of newer antibiotics has paved the way for considering the use of older antibiotics such as Fosfomycin. This study aims to determine the in-vitro susceptibility of uropathogens to Fosfomycin. Materials and Methods: In the present descriptive study conducted over a period of 6 months in a tertiary health care center of South India, 1000 urinary samples were processed. The isolated uropathogens were identified and tested for susceptibility to routine urinary antibiotics. Along with this, the in-vitro activity of Fosfomycin was detected by disc diffusion and E-test. Results: Out of the 1000 urine samples processed, 200 uropathogens isolated were included in the study. E. coli was the predominant (51.77%) Gram negative isolate and Enterococci was the commonest (61.02%) among Gram positive organisms. Fosfomycin was the most sensitive antibiotic (84.49%) compared to other urinary antibiotics tested. Conclusion: Fosfomycin has a high in-vitro susceptibility against most of the uropathogens except Enterococci. So we recommend its use as empirical first line oral therapy for uncomplicated UTIs.
