Free Fibula Flap for Bone Lost Complicated with Recalcitrant Osteomyelitis
This article presents a compelling clinical case of a 36-year-old male with a severe gunshot injury resulting in significant tibial bone loss and recalcitrant osteomyelitis. After failed conventional treatments, successful reconstruction was achieved using a **free vascularized fibula flap**. The case highlights the dual advantage of this technique: it provides robust structural support to bridge large bone defects and, uniquely, enhances infection control by delivering a rich, independent blood supply that carries antibiotics directly to the infection site. Follow-up at seven months confirms excellent bony union and graft hypertrophy, demonstrating the flap's superiority in managing complex, infected bone non-unions.
